Colorado 211 maintains relationships with organizations across the Western Slope providing health and human services programs. It is through these relationships that 211 is able to provide accurate and comprehensive information and referral services to individuals and families across all different backgrounds and financial situations.
The Colorado 211 database contains detailed information regarding agencies and their services beyond just a short description. Information including:
- Location
- Eligibility requirements
- Intake procedures
- Counties/Cities/Areas served
- Languages spoken
- Fee information
- Contact information
- Hours of operation
- And more
Colorado 211 helps service providers and clients avoid the frustration caused by innaccurate and outdated
referrals. Our database is dynamic and our Resource Department works continuously to maintain current,
accurate and consistent information.
New Agencies
If your organization’s information is not within the 211 database and you would like to be included, please fill out the following forms below detailing your agency and the services it provides.
In our efforts to ensure that the needs of the community are met, Colorado 211 has developed criteria for the inclusion or exclusion of agencies, organizations and programs in the 211 database. These criteria are uniformly applied and published so that staff and the public are aware of the scope and limitations of the 211 database.
If you are a new organization not listed within the 211 database and would like to submit your information to us, please read through the Inclusion/Exclusion Policy below and verify that you have read and understood the criteria. If you are unsure if your organization qualifies to be a part of 211, please email us at wc211@wc211.org or call us by dialing 2-1-1.
Once both the Agency Form is complete and you have read and signed the Inclusion/Exclusion Policy, you can submit both forms to us via email at wc211@wc211.org. You can also print and mail to us at 1129 Colorado Ave, Grand Junction, CO 81501.

If your organization’s information is already listed in the 211 database and you have updates on changes to agency or service operations that you would like to submit, please fill out the form below.
